These California Privacy Disclosures supplement our Privacy Statement and apply to residents of California. They are provided under the California Consumer Privacy Act, as amended by the California Privacy Rights Act (the "CCPA"), to the extent the CCPA applies to Jellyphysh LLC ("Jellyphysh," "we," "us," or "our"). Capitalized terms not defined here have the meanings given in the CCPA.

We collect this information from you directly and automatically when you use the Website, for the purposes described in Section 4 of our Privacy Statement. We keep each category only as long as reasonably necessary for those purposes, as described in Section 12 of our Privacy Statement.
2. Disclosures for a Business Purpose. In the past 12 months, we have disclosed identifiers, internet activity, and approximate geolocation to service providers, such as website hosting, email and text message delivery, analytics, and customer support providers, for business purposes, including operating the Website, communicating with you, and security. When you give us your name and email before starting an intake, we also disclose them to CareValidate, Inc. at your direction so it can begin the consultation you requested.
3. No Sale or Sharing. We do not sell personal information, and we do not share personal information for cross-context behavioral advertising. We have not done so in the past 12 months. We do not knowingly sell or share the personal information of consumers under 16.
4. Sensitive Personal Information. We use sensitive personal information only for purposes permitted by the CCPA, such as providing what you requested and maintaining the security of the Website, and not to infer characteristics about you. Because of this, we do not offer a separate right to limit its use.

We will verify your identity by matching the information you give us to information we already have. You may use an authorized agent, but we may require proof that the agent is authorized and ask you to verify your identity directly with us. We will respond within 45 days, which we may extend by an additional 45 days with notice to you.
7. Global Privacy Control. We honor Global Privacy Control signals as a valid request to opt out of the sale or sharing of personal information for the browser or device sending the signal.
8. Medical Information. Medical information you provide in the intake, patient portal, or telehealth visits is collected and maintained by the medical group assigned to you, not by Jellyphysh, and is governed by HIPAA, the California Confidentiality of Medical Information Act, and the medical group's Notice of Privacy Practices.
9. Shine the Light. We do not disclose personal information to third parties for their own direct marketing purposes.
