This Privacy Statement explains how Jellyphysh LLC ("Jellyphysh," "we," "us," or "our") collects, uses, and shares information about you when you visit jellyphysh.com (the "Website"), sign up for our emails or text messages, contact us, or otherwise interact with us. 1. Who We Are. Jellyphysh is a marketing and referral company. We are not a healthcare provider, pharmacy, or health plan, and we do not provide medical care. When you choose to pursue treatment, you leave our Website and enter a secure intake and patient portal operated by CareValidate, Inc. ("CareValidate"). Medical services are provided by independent licensed providers of the medical group assigned to you (the "Medical Group"), and prescriptions are filled by independent licensed pharmacies (the "Pharmacies").
2. What This Statement Does Not Cover. This Privacy Statement covers only information that Jellyphysh collects. It does not cover the health questionnaire, intake, checkout, patient portal, telehealth visits, prescriptions, or laboratory services. Information you provide in those places is collected and handled by CareValidate, the Medical Group, and the Pharmacies under their own privacy policies and, for the Medical Group, its Notice of Privacy Practices, which is available in the patient portal. Please review those documents before you begin an intake.
Jellyphysh does not receive your intake answers, medical history, medical records, prescriptions, laboratory results, or other health information you provide through the intake or patient portal. We may receive de-identified or aggregate reports from CareValidate, such as the number of new sign-ups or orders by state, that do not identify you.
3. Information We Collect.
Information you give us. When you sign up for our emails or text messages, give us your name and email before starting an intake, fill out a contact form, or reach out to customer support, we collect the information you provide, such as your name, email address, phone number, your consent preferences, and the content of your message. If you give us your name and email before starting an intake, we pass your name and email to CareValidate so it can begin the consultation you requested. Once you enter the CareValidate intake, we do not have access to the information you provide there. Please do not send us medical or health information by email, text, or contact form. Clinical questions should go to your provider through the patient portal.
Information collected automatically. When you visit the Website, we and our service providers may automatically collect information about your device and how you use the Website, such as your browser type, IP address, approximate location derived from your IP address, pages viewed, links clicked, the site that referred you, and the date and time of your visit. We collect this information using cookies and similar technologies.
Information from others. We may receive aggregate campaign performance reports from advertising and social media platforms, and de-identified or aggregate reports from CareValidate as described in Section 2.

Under some state laws, information that suggests you are interested in a health treatment, such as the fact that you visited a treatment page, gave us your name and email before starting an intake, or signed up for related emails, may be treated as consumer health data. 7. Text Messages. If you opt in to receive text messages from us, we collect your name, phone number, and your consent to receive messages. We use this information only to send you the messages you signed up for and to operate our messaging program. Mobile opt-in information and consent are never shared with third parties or affiliates for marketing or promotional purposes, and any sharing described elsewhere in this Privacy Statement excludes mobile opt-in data.
